fa بررسی فنوتیپی و ژنوتیپی شیوع ژن های بتالاکتاماز طیف گسترده TEM، SHV و CTX-M در سویه های E.coli جدا شده از نمونه های بالینی بیماران بستری در تهران به روش PCR Phenotypic and Genotypic Analysis of the Prevalence of TEM, SHV, and CTX-M Extended-Spectrum Beta-Lactamase Genes in Escherichia. coli Strains Isolated from Clinical Samples of Hospitalized Patients in Tehran Using PCR میکروبیولوژی پزشکی پژوهشي Research در سال های اخیر، افزایش سویه های اشرشیاکلی تولید کننده ی بتالاکتاماز های طیف گسترده (ESBLs) ، منجر به محدود شدن گزینه های درمانی شده است. این مطالعه ی بر روی 100 ایزوله ی اشرشیاکلی در طی مدت 3 ماه انجام شد و حساسیت باکتری های جدا شده نسبت به 7 نوع آنتی بیوتیک با روش انتشار دیسک دیفیوژن بر اساس دستورالعمل استاندارد CLSI 2021 ، تعیین شد و سویه ها با روش دیسک ترکیبی  (CDDT)  از نظر وجود آنزیم های بتالاکتاماز طیف گسترده بررسی شدند. ژن های CTX-M, SHV و TEM با روش PCR شناسایی و تعدادی از ایزوله ها به روش توالی یابی سنگر ، تعیین توالی و در پایگاه داده NCBI، بلاست گردید. از 42 ایزوله ی اشرشیاکلی جداسازی شده، مقاومت به آنتی بیوتیک های آمپی سیلین، سفتازیدیم،سیپروفلوکساسین، ایمی پنم، نالیدیکسیک اسید، سفتریاکسون و جنتامایسین به ترتیب برابر با 5/90%، 7/85 %، 5/59 %، 4/52 %، 5/40 %، 7/35 % و 5/9 % بودند. 23 ایزوله( 8/54 %)  ESBL مثبت، و از بین آن ها 3/78 % ایزوله ها دارای ژن CTX-M، 9/73 % ایزوله ها دارای ژن TEM و 7/21 % ایزوله ها دارای ژن SHV بودند. فراوانی سویه های اشرشیاکلی مولد ESBL، در این مطالعه نشانگر این است که تولید ESBL و شیوع آن از شهری به شهر دیگر متفاوت است. بنابراین استفاده از روش های آنتی بیوگرام ، تشخیص مولکولی و شناسایی انواع ژن های بتالاکتاماز طیف گسترده، باید به صورت دوره ای در تمام مراکز درمانی و بیمارستان ها انجام شود.   In recent years, the increase in Escherichia coli strains producing extended-spectrum beta-lactamases (ESBLs) has limited treatment options. This study was conducted on 100 E. coli isolates over a period of 3 months. The sensitivity of the isolated bacteria to 7 types of antibiotics was determined using the disk diffusion method according to the CLSI 2021 standard guidelines. The strains were examined for the presence of ESBL enzymes using the combined disk test (CDDT). The CTX-M, SHV, and TEM genes were identified using PCR, and some isolates were sequenced using the Sanger method and blasted in the NCBI database. Among the 42 isolated E. coli strains, resistance to ampicillin, ceftazidime, ciprofloxacin, imipenem, nalidixic acid, ceftriaxone, and gentamicin was 90.5%, 85.7%, 59.5%, 52.4%, 40.5%, 35.7%, and 9.5%, respectively. Out of 23 isolates (54.8%) that were ESBL positive, 78.3% carried the CTX-M gene, 73.9% harbored the TEM gene, and 21.7% had the SHV gene.The prevalence of ESBL-producing E. coli strains in this study indicates that ESBL production and its prevalence vary from city to city.
